It is very important for Fairfield City Schools to document all positive COVID cases that impact our students, faculty and staff members. The information has to be reported to the state and it helps our Superintendent make informed decisions regarding virtual instruction.
Please complete one form per person if a Fairfield City Schools’ employee or student has tested positive for COVID. If you have questions, please email zodoms@fairfieldk12.org
